Per hospital we take the median of all in-network negotiated_dollar rows for the selected CPT and canonical payer. When a row reports a percentage of gross charge instead of a dollar amount, we derive the dollar value via percentage × gross_charge ÷ 100.

TiC fallback uses negotiated_type = 'negotiated' only — percentage and per-diem rates aren't directly comparable to MRF dollars.
